GP Factual Reports PIP
Can anyone provide me with a blank copy of a GP facutal report form
I am presently writing a submission in a case where the HCP states on the PA4 that
” Miss ... has no behavioural problems as reported in the GP factual report”
The GP factual report is not in the bundle but I am well aware that Q7 on the reort form asks ” Does the patient have a history of violent or threatening behaviour”
My client has no such history ,she has learning difficulties and anxiety and depression.
It is my view that the HCP has distorted the factual report.
The GP factual report is now over 4 years old so the DWP migh have got rid of it by now, although the PA4 only dates from 2021
