DLA 434 (Child) form?

I’m homeworking. My client has got a DLA 434 (Child) to fill in. I normally do this sort of thing over the phone, completing the form on my laptop, getting it printed out and sent to the client for signature and sending on, along with the original. But I can’t find a DLA 434 anywhere. Does anyone know where I can get hold of a digital version that I can download or complete online?

Is the DLA 434 (Child) nearly the same as DLA1A (Child). I have easy access to this. Would I be okay using this?

Any other suggestions or work-arounds?
