Adult DLA change of circs form

Hi,
Does anyone have a link or copy of a DLA change of circs form I can use? I can only find DLA for children online.

Supersession requests for adult DLA are treated as PIP claims. Or did you need the form for some other reason?

Unless older enough to stay on DLA (born on or before 8th April 1948).

yes she is older and stays on DLA. I was hoping to do a long distance form completion for a family member but it looks like I will just have to go for a visit once they ask for review forms from DLA.

Occurred to me to mention that, but then I noticed the title said Adult, rather than Pension age DLA :)

EDIT: Below not relevant after seeing OP’s last post.
(Also noticed you’re in Edinburgh Annie, so it will be ADP, rather than PIP. Though I assume you already knew that bit. )

yes I put adult in to differentiate between the child DLA form, which you can get online, but she is a pensioner who was born in 1940 so she will be a DLA review.

I assisted an elderly family member recently who sought a review in care needs and they had phoned up asking for a review form and they simply received a complete DLA pack to complete. The review was successful, mind you only only after the most basic of MR request!

I know the form number is DLA434 but can’t find it online either. An FOI request would work but probably not gonna help you in time I suspect.

There was a FOI back in 2012, which turned up this: https://www.whatdotheyknow.com/request/electronic_dla434_october_2011

I don’t know that that’s the current version.

(Does Daphne use a male alter-ego for FOIs? : ) )
