Having gave up on trying to make a claim over the phone and difficulties getting online for non computer savvy clients does anyone have a paper JSA claim form??

This is the start of many JSA claims due to MR :(

I previously attached an old scan, at http://www.rightsnet.org.uk/forums/viewthread/3604/ . It seems the CAB have now withdrawn this from our info system, so I don’t know if it’s still an approved form to use or not. The CAB info doesn’t seem to mention claiming JSA on paper at all, though CPAG confirm it’s still allowed.

Many thanks, I can see no reason why they would refuse to accept a paper claim, we’ll see how it goes 😊
