Guide to completing DLA & AA forms

We are in the process of drafting a brief guide to completing DLA & AA forms, which we can hand out to those of our clients who are able to complete the forms themselves with a bit of preliminary advice from us. Are there any organisations out there who already have something similar, and if so, could we have a look?

At the Nattional Deaf Children's Society we have produced guides to filling in DLA forms for Deaf children and young adults. these are available on line on http://www.ndcs.org.uk/information/financial_support/dla_a_guide_to.html
i have also found Cerebra's guide to be helpful for children with additional needs. www.cerebra.org.uk
