As many of you will know the DWP are piloting a shortened DLA claim form at Wembley DBC. Whilst many of us would no doubt welcome any steps which reduce the length of DLA claim forms, not at the expense of claimants losing out on benefit. From a brief glance I have a number of concerns about the claim form being used. I understand that the decision to introduce the new style of claim form has already been taken and there appears to be no widespread formal consultation process in relation to this.

Both Disability Alliance and the RNIB were inviting comments on the form and so if you have any concerns they could be sent via them. A copy of the claim form is available by e-mailing Disability Alliance.

Have just spoken the Disability Alliance and they have suggested that rather than emailing them you can view a copy of the pilot form on the Disability Alliance website at -

Note though that you cannot print the form and use it to make a claim!
