Content of written reasons/record of proceedings

Below is a query from a colleague in another LA. Thought I'd put it in a bigger forum - she doesn't have access to the net

'You know when you get the full writtens, and there is the record of proceedings with it, well, does it have to contain certain things? I know that it can be in a format that the chair feels is appropriate, but I have one which has been typed up, with no mention of clients name, U no. or NI No. nor does it say who was present at the hearing. It arrived with the full decision. I have put the request for leave to appeal in on different grounds, but am wondering if this could be a ground too, you know like if you can't read the R.o.P.'
